Authorization for a Third Party to Select a Natural Gas Supplier
If you are a Columbia Gas of Pennsylvania customer, you and anyone you designate may make decisions regarding the selection or change of your natural gas supplier. To do so, you must provide us with a signed document, identifying by name, all those who have the authority to initiatie a change in your natural gas supplier.
Columbia Gas of Pennsylvania has developed a simple form customers can use to authorize another party* to change their natural gas supplier.
The form requires any customer who wants to pursue this option to provide:
- The account holder's name;
- PCID number;
- Zip code plus four digit extension;
- The account holder's signature;
- Date of request; and
- The name of individual being authorized.
If you prefer not to use the form provided here, you may send a letter including all of the above information to us requesting that another individual(s) be given authority to initiate a change in your natural gas supplier. Send the letter to:

NOTE: Cancellation of an authorization agreement shall require in writing the following: account number, zip code plus four-digit extension, signature of account holder, date the request is being made and the name of the person being removed.
